It enters the skin underneath the collar bone, passes through the actual body cavity beneath the bone, and emerges on the other side. Maybe not the most radical “beauty” treatment here but let me add to the horror… A friend of mine fell asleep on the beach with these. Instead of just bad sunburn she spent 3 hours in ER getting the burning piercings surgically removed. The resulting scars are horrendous.
